Patrick Roger Forthman, 45, of San Bernardino, CA. passed away on Tuesday, March 29, 2016 in San Bernardino. He is survived by his mother, Louise of San Bernardino, brothers, Michael, Timothy, William and Keith Forthman and an Aunt Mary Szalai Conneaut, Ohio and many friends. Visitation will be from 5 to 7 p.m. on Thursday, April 14th at Bobbitt Memorial Chapel, 1299 E. Highland Ave. San Bernardino with graveside services on Friday April 15, 2016 at 2:01p.m. at Riverside National Cemetery, 22495 Van Buren Blvd. Riverside
